Antonio Conte’s men defeated West Brom by 1-0, with Michy Batshuayi scoring the solitary goal, and won their manager the title in his first season in the Premier League.
Chelsea have been brilliant through out the season, with all their big names delivering for them with the bench also coming good. Conte has managed to have a fit squad for almost the entire season, and with players like Willian, Cesc Fabregas and Batshuayi coming off the bench, the squad looked ominous for their rival teams.
Chelsea have been blessed to have players who gave their blood and sweat for their team, and were rewarded with the title. N’ Golo Kante has been the player of the season, and big names like Diego Costa and Eden Hazard have also been sensational. New signings David Luiz and Marcos Alonso have adapted to the Premier League like a fish to water.

The Belgian goalkeeper struggled last season under Jose Mourinho, but his back to his very best this season, as he played a major part in Chelsea’s title win. Courtois has kept 16 clean sheet this season and has made a number of vital saves, some of which have been instrumental in keeping Chelsea at the top of the table. His stunning save at Sunderland stood out, and earned him plaudits from experts and fans.
The former Atletico Madrid goalkeeper is targeted by Real Madrid, and Chelsea will have to make sure they keep hold of their prized asset if they are to retain the Premier League title, and have a shot at the Champions League as well. Courtois is one of the finest goalkeepers in the world, and Chelsea should keep him happy.
2) Cesar Azpilicueta: 8
The Spaniard has played in multiple positions this season, but played majorly at Centre back and has thrived in the three man defence alongside Cahill and Luiz. ” Dave”, as he is fondly called, has been one of the finest defenders in the league and has been an ever present in this title winning Chelsea side. The Spainish defender is extremely sound defensively and does not give much to the attackers.
Azpilicueta’s versatility also stood out, and he has become a favourite of manager Antonio Conte. He does not attract a lot of headlines, but does his job brilliantly and is a manager’s delight. Chelsea would be hoping he continues his form next season as well and win more laurels with the Blues.
3) David Luiz: 9
From being called a panic buy to becoming one of the best defenders in the league, David Luiz has come a long way. When Conte signed the Brazilian from PSG in the summer, lots of eyebrows were raised as Luiz was too error prone. But the Brazilian has answered his critics in style as he has been one of the best defenders in the league this season, if not the best.
Luiz has been a nightmare for forwards as he has given nothing away at all, and has been solid as a rock in the centre of the three man defence. He was also included in the team of the year, and credit goes to Antonio Conte for the way he has handled him, and transformed him into one of the best defenders in the world. Luiz has been playing with a knee injury for quite some time now and he deserves utmost credit for his performances.
4) Gary Cahill: 8
Captain fantastic, Cahill also suffered due to lack of form last season, but has been brilliant this season and has also thrived in the three man defence deployed by manager Antonio Conte. Cahill has been rock solid in defence, and has also chipped in with a few vital goals.
The Englishman has marshalled the defense well, and has led from the front in what has been a fantastic season for the Blues. Cahill has also been included in the team of the season, and would be looking to Maintain his good form next season as well.

Like Luiz, the Spaniard’s signing raised a few eyebrows, but he has been a revelation this season for Antonio Conte’s side, and has contributed immensely in defence as well as attack. Alonso has been an ever present in the eleven, and enjoys the trust of manager Antonio Conte.
The Spaniard wasn’t the one who gathered all the attention, but did his work superbly as he played a major part in Chelsea’s title win. He was sound in defence and also scored 6 goals, which is a good output for a left wing back. He would be looking to Maintain his good form next season as well.
6) Victor Moses: 8
Who would have thought Victor Moses would play a huge part in a Chelsea title win. Not trusted by Jose Mourinho, the Nigerian was shipped out to various clubs on loan. But Antonio Conte came in and gave him his chance, and Moses did not disappoint as he was impeccable at right wing back throughout the season. Moses, like Alonso has also been good in both defence and attack, and has chipped in with a few goals and assists. Chelsea’s formation suited him,and he has been one of their best players. Conte deserves for the way he has transformed the Nigerian.

The Serbian’s time at Stamford Bridge looked to be coming to an end with the signing of Kante, but manager Antonio Conte preferred to play both of them in the middle of the park, and Matic has excelled. The midfielder has been very good in the middle of the park alongside Kante and has also added an attacking threat to his game, as he has provided assists and scored a few belters as well. Matic provides a different dimension to the Chelsea team and has been impeccable in Chelsea’s title win.He would be looking to Carry his good form next season and guide Chelsea to more title glory.

Pedro struggled to find his best form in his first season in the Premier League, but has been absolutely brilliant this season as he has scored goals, provided assists and been a headache for defenders. Pedro struggled for game time at Barcelona, because of the presence of MSN, and was snapped up by Jose Mourinho for Chelsea. He has replicated his Barcelona form this season, and has been preferred ahead of Willian by Conte. He would play a major part next season as well, as he has the experience of playing in the Champions League.
10) Diego Costa: 9
The fiery striker has had his issues, but still has 20 league goals to his name as he has driven Chelsea towards the title with his goals. Costa was involved in a training ground row with manager Antonio Conte, but the two have made up and the Spaniard has performed admirably to deliver the title for his team.
He might be hated by some, but there is no doubt he is a gifted footballer, and has now won two PL titles in three years. Costa is expected to leave Chelsea at the end of the season and if that happens, then the Blues need an apt replacement for him as he guarantees goals.
